Be Well Health Group has put together this privacy statement to provide you with knowledge and peace of mind in regards to our commitment and care when managing your personal information. This includes the information you provide to us upon initial contact, as well as the ongoing personal and health-related information that you share with your practitioner.

We are committed to providing safe, quality, holistic and individualised care. To help ensure this level of care we are required to collect personal information. Our team takes your privacy seriously; your personal information is handled with respect and confidentiality in accordance with our privacy policy, which follows the Australian Privacy Principles (APP) in the Privacy Act 1988. 

Information we collect and why:

  • Personally identifying information, such as name, date of birth and your medicare number.

  • Assists with identification when corresponding with third parties (GPs, private health providers, Medicare, and other third party providers i.e WorkSafe, TAC etc.) with your prior consent. 

  • To enable us to process payments.

  • Contact details.

  • To enable us to get in contact with you in regards to your appointment, to follow-up after treatment, to relay other information, such as exercises, medical certificates, and to inform you of any changes happening at the clinic (you may unsubscribe from our email updates via the ‘unsubscribe’ link at the bottom of the email).

  • Emergency contact details are important in case of emergency, for example if your condition is worse than initially thought, an ambulance may be required, or if you find that you are unable to drive following treatment (occasionally treatments may leave some people feeling light-headed, for example).

  • Lifestyle and health-related information.

  • Assists your Osteopath with assessing your situation and potential contributing factors involved with the conditions that you’re presenting with.

  • Allows your osteopath to establish an appropriate, individualised treatment plan with you.

How your information is stored and staff access:

  • Security is our practice management software provider’s top priority, and they strictly follow the requirements of the Australian Privacy Principles (APPs). 

  • Each staff member’s access to information is restricted depending on their role, and requirement for access in order to perform their role.

Disclosure (sharing of information):

  • Occasionally, in order to provide services to you, we need to disclose your personal information, such as when liaising with another practitioner you are also receiving treatment from. This information will only be disclosed:

  • With your consent.

  • When required by law, for example, if it relates to any legal proceedings involving an insurance claim being made following an accident.

  • Where necessary to enhance your treatment outcomes; again, only with your consent.

  • Although we do our best to maintain the security and privacy of your information, you also need to be aware that:

  • The internet is an unsecured public network.

  • There is a risk that your transactions (including emails) may be seen, intercepted or modified by third parties.

  • Downloadable files may contain computer viruses, disabling codes, worms or other devices or defects.

Accessing and correcting your information:

  • You have the right to access your personal information, and may make a request to us to do so. In line with the Health Records Regulations 2012, there may be a fee associated with your request - this fee will be dependent on the nature and amount of information you have requested.

  • You may also request the correction of any personally identifiable information.

Your consent:

By attending and engaging with the services provided by Be Well Health Group, you consent to our collection and use of your personal information as described within this Privacy Policy. We reserve the right to amend this privacy policy at any time with or without notice. 

Complaints:

If you believe that we have breached the Privacy Act or mishandled your personal information, please contact us in writing (post or email). Please provide as much detail as possible so that we may investigate the issue and respond as appropriate.
