Understanding Chronic Condition Management Plans
A Chronic Condition Management (CCM) Plan, formerly known as an Enhanced Primary Care (EPC) Plan, is a Medicare initiative that allows eligible patients to receive rebates for allied health services following a referral from their GP.
CCM Plans are generally available to individuals who have been managing a chronic condition or persistent pain for six months or longer. If you are eligible, your GP can prepare a CCM Plan to help support your ongoing healthcare needs and connect you with the appropriate allied health professional/s.
Patients are eligible to receive a maximum of 5 allied health sessions across all allied health professions within a calendar year.
These sessions can be used across a range of eligible allied health professions, including:
Osteopathy
Physiotherapy
Exercise Physiology
Podiatry
You can choose how to use your sessions depending on your individual healthcare needs. For example, you may choose to use 3 sessions with an Osteopath and 2 sessions with a Physiotherapist, or you may decide to use all 5 sessions with an Osteopath.
Using Your Chronic Condition Management Plan at Be Well Health Group
At Be Well Health Group, patients can use their CCM Plan with any of our Osteopaths.
Osteopathy can play an important role in helping people manage a range of chronic conditions, including persistent back pain, neck pain, headaches, arthritis, joint pain, and mobility concerns. Treatment focuses on improving movement, reducing discomfort, and supporting overall function so you can continue doing the things you enjoy.
Please note that Medicare rebates under a CCM Plan are currently only available for Osteopathy services at Be Well Health Group. Our Myotherapy, Remedial Massage Therapy, and Chinese Medicine practitioners are unable to accept CCM referrals.
How Your Payment Will Be Processed
Patients will be required to pay the full amount of their Osteopathic service upfront.
Our reception team will then be able to process your Medicare rebate (currently $61.80) on your behalf.
What To Bring To Your Appointment
To ensure your appointment runs smoothly, please bring:
Your Chronic Condition Management Plan from your GP (if it has not already been provided to us).
Your Medicare card.
A physical bank card with a savings or cheque account (this is essential for processing your Medicare rebate).
Keeping Track of Your Appointments
As Medicare only funds 5 allied health sessions per calendar year, patients are encouraged to keep track of the sessions they have used throughout the year.
Patients will be expected to pay in full for any sessions that fall outside of these 5 sessions.
Patients can easily monitor their available sessions through their Medicare Online account or the Medicare app.
